A questionnaire of your complete medical history, vital signs, urine sample, list of all doctors, diagnoses, and list of medications with dosage. All information is sent directly to your insurance company.
A blood sample taken from your arm or finger, using only sterile, disposable needles and supplies. Samples are sent to a laboratory designated by your insurance company, with results sent directly to your insurance company.
A drug test that is regulated by the government, such as the Department of Transportation. We collect the urine sample and send it to the laboratory of your company’s choosing.
